For your convenience, we have made our forms available online. Due to HIPPA rules and regulations, these forms must be completed prior to starting therapy. If you wish to fill them out in advance, the links will open each one in a new tab in your browser. Please print, fill out, and sign both the Intake Form and Informed Consent forms, and bring both of them with you to your first visit.
